Sponsored work permit
These are arranged by an employer once a position has been offered and accepted by the candidate. There will be a contract drawn up between the employer and candidate, by negotiation. These visas are usually valid for 2 years, but are only valid for that specific employer (ie a specific hospital or group of hospitals if an NHS Trust or private group). Once an employee, if you wish to resign from a specific employer, you will need to gain another sponsored work permit from another employer before changing jobs.
Leave to Remain/Enter
To work in the UK as a Work Permit holder you will also require valid Leave to Enter or Leave to Remain. This is your permission to actually be in the UK.
If you are currently outside the UK, you will need to apply for entry clearance (leave to enter) before you travel. The entry clearance process for the UK is run by UK Visas through the British diplomatic posts around the world, more information is available on their website http://www.ukvisas.gov.uk/.
If you are already in the UK you will need to apply for Leave to Remain or a variation of Leave to Remain. Applications are completed by post and are sent to the Home Office in the UK. More information is available on their website. http://www.bia.homeoffice.gov.uk/workingintheuk/
There is an application fee that applies to both Leave to Enter and Leave to Remain applications, further information can be obtained from the appropriate authority.
