About our Case Management Service

Increased choice, independence and general health and well being are seen to improve as a result of physical, emotional and psychological needs being met. Our care solutions can be a flexible mix of all or any of the services below, whether they need long term care, or simply assistance to recover from an accident or hospital stay.

Continuing Care

We can provide complete nursing teams, or more cost effective Nurse led HCA teams, who can operate under the delegation of tasks.

Social Care and Support

Our social care and support workers can be provided to prompt and assist individuals to live as independently as possible, enabling service users to access activities in their community and have confidence and reassurance when undertaking day to day tasks.

Personal Care and Companionship

Provides assistance with more personal care needs, like getting ready in the morning, going to bed at night, showering, bathing or dressing. Alternatively your client may just require companionship and the reassurance of someone to talk to during the day.

Nursing Care

Nursing Care can range from simple regular dressing changes, medication or injection services, through to post-operative or palliative care. More complex needs can be catered for, using skilled nursing teams and carers via delegated tasks.

Live-in Care

Whilst our services are available on a visit basis, our live-in care packages would be more suitable if your client required 24 hour care, day and night. In these cases our team would effectively live in your clients home and deliver twenty four hour cover.
