It is for this reason that, where appropriate for the service user and client, we are able to use carers to deliver care under the delegation of tasks process. Under a carefully supervised training and task delegation process, our Continuing Care team implement and manage the complex care needs of these patients, providing bespoke specialised training and on-going supervision to all of our Healthcare Professionals. The allocation of a lead nurse, responsible for providing on-going supervision and ensuring that delegated task competencies are continually assessed, ensures patient focus outcomes are achieved whilst providing a cost effective solution at all times. This model ensures the service user benefits from continuity of care by a dedicated team, trained in their specific needs with additional support from a proven team of Continuing Care specialists.
